Pretty much all four-armed humanoids I’ve seen simply have four regular arms. This is fine, but there are a lot of other ways you could go with it It seems the common factor among humanoid creatures is brachiation. All real creatures I know of that resemble the humanoid form are either themselves regular brachiators or evolved from an animal that brachiated. Real, two-armed brachiators often feed in a particular way; they hang under a branch by one arm, and use the other arm to reach for food. Now, what would happen if one of these brachiators had an extra pair of arms? It likely couldn’t hang by the lower arms, as they would get in the way of the upper arms if it tried to reach with them. And it would be more useful for both upper arms to be used for grip; 1-hand grip → 2-hand grip is doubled grip, while 2-hand reaching → 3-hand reaching is only 50% better at most If we adapt this form to a mostly terrestrial walker, the most reasonable result would be an upper pair of arms for balancing while walking, which are stronger and have human-like shoulders but with less dexterous hands, and a lower pair of arms for manipulation, which are weaker and have narrower less-prominent shoulders but with highly dexterous hands that have thumbs and such
